Requirement to Receive/Documents:

PWD/itself:

1. Original PWD ID (2xeroxcopy)(Front and Back)(put a thumbmark on xerox copy)

2. Birth Certificate (2 xerox copy)

Representative: (together with those documents of PWD)

1. Valid ID (Government Issued ID) (Student ID is not valid)

EDUCATIONAL ASSISTANCE FOR PWD

Requirement to apply:

1. Certificate of Indigency (1 original copy and 1 xerox copy)(be specific the reason of applying is for EDUCATIONAL ASSISTANCE ONLY)

2. PWD ID (2 xerox of front and back do not put a handwritten signature on it)

3. Certificate of Registration (2 xerox copy)

4. Certificate of Grade last school year

5. Sedula (2 xerox Copy)

FINANCIAL/MEDICAL ASSISTANCE FOR PWD

ONLY for those PWD who have mental/ intellectual disability like:

Hydrocephalus

Mental Retardation

Cerebral Palsy

Down Syndrome

Epilepsy

NATIONAL DISABILITY PREVENTION AND REHABILITATION WEEK

Assistive devices are being distributed (e.g., ECG monitors, canes, crutches, wheelchairs, etc.).

Walker(10 pcs)

Walking Stick(10 pcs)

Quadcage(10 pcs)

Requirements:

1. Full bod picture of PWD without any Assistive device together with a cardboard writing a FULL NAME, BARANGAY, REQUESTED ASSISTIVE DEVICE

TRAINING ON BASIC CAREGIVING FOR PWD

36 slots for trainees/ care giver either a family member or non-family member
